The California Quality Collaborative’s first annual network convening brings together a diverse group of stakeholders to align initiatives and develop solutions for a more equitable and effective health care delivery system. This dynamic event features engaging presentations by leading experts in the field, networking opportunities for attendees to build valuable relationships, panel discussions exploring solutions to critical systemic issues and a partner showcase highlighting innovative initiatives and collaborations from CQC partners.

Actionable Strategies for Primary Care Investment: Leveraging Alternative Payment Models

This session explores how to develop and implement strategic plans for increasing investment in primary care, with Alternative Payment Models (APMs) as a key component. While APMs present both challenges and opportunities for payers and the provider networks, APMs, and specifically hybrid models, can be strategically utilized to drive investment in primary care by demonstrating value, managing risk, and incentivizing high-quality, cost-effective care. Panelists will share practical insights and experiences in developing comprehensive primary care investment strategies and successfully implementing hybrid APMs by payers and providers.

California Data Exchange Framework Town Hall

Location: Breakout Room 1 (Academy 6)

Join CDII Director John Ohanian and data exchange leaders for an interactive Town Hall designed to gather direct feedback from providers, health systems, and other key stakeholders on the implementation of California’s Data Exchange Framework (DxF). As California advances efforts to create a more interoperable, connected health and social services ecosystem, your input is critical in shaping how data exchange can best serve the needs of patients, providers, and communities.

Director John Ohanian will present on the progress of the DxF to-date and the DxF Three-Year Roadmap followed by a conversation with Anwar Zoueiheid, Partners in Care Foundation on how Los Angeles partners leveraged the DxF Community Sandbox to advance interoperability for Enhanced Care Management (ECM). Participants will have the opportunity to ask DxF questions and share feedback on successes, challenges, and lessons learned related to data sharing implementation.

This session will focus on practical strategies for scaling community-based practice coaching programs within payer and provider organizations focused on reducing health inequities. Panelists will share their experiences in developing sustainable models, training coaches on culturally competent care and addressing health-related social needs, leveraging community partnerships, and measuring the impact of scaled coaching initiatives on disparity reduction.

Catalyzing Integrated Care in California

With California’s health care system at a critical juncture as primary care teams support patients’ increasing behavioral health care needs, innovative solutions are essential for payment, partnership and service delivery. This plenary session will explore opportunities to catalyze behavioral health integration (BHI) across the state, examining key levers in implementation and financing. This session, moderated by Kristina Mody of California Quality Collaborative, will:

  • Examine Behavioral Health Needs and Service Utilization: Covered California’s Dr. Charles Raya, Equity & Quality Specialist, will provide analysis of key data trends to understand current utilization patterns and identify areas and populations of need.
  • Highlight Successful Implementation Models: Hear firsthand from Jiami Wu, Director of Clinics of Chinese Hospital, a nationally recognized CalHIVE Behavioral Health Integration (BHI) participant, about their implementation progress and experience with payment for integrated services.
  • Share BHI Financing Recommendations and Payer Implementation: Explore CQC recommendations for BHI financing and discuss the 2025 Payer Workgroup objectives with perspectives from Dr. Jennifer Christian-Herman, Vice President of Behavioral Health and Clinical Strategy at Blue Shield of California, outlining a roadmap for future payment reform.
  • Explore Best Practices from Other States: Learn from Collaborative Family Healthcare Association’s Dr. Neftali Serrano, Chief Executive Officer, about successful behavioral health payment approaches implemented in other states and consider their potential applicability in California.
  • Address Barriers to System Change in California: Participate in an open discussion to identify and address key barriers hindering progress in transforming California’s behavioral health system.

Keynote Speaker

Dr. Mark Ghaly

Dr. Mark Ghaly was appointed Secretary of the California Health and Human Services Agency in early 2019 by Governor Gavin Newsom and recently stepped down after nearly 6 years of service. In serving California, Dr. Ghaly had the privilege of working with partners across diverse sectors and disciplines to improve the lives and life chances of ALL Californians. This was especially true during our response to the COVID-19 Pandemic but spanned his entire tenure. Dr. Ghaly’s experience as a community-oriented, primary care pediatrician allowed him and his team to work across California’s health care and social services safety net systems, to promote and advance a whole person, whole community, approach to integrating services. He and his team worked to ensure that CalHHS services and programs are equity-anchored and person-centered for ALL, but especially the most vulnerable Californians. Dr. Ghaly holds the deep belief that together our collective efforts can not only serve Californians well today but set us on a path where ALL Californians have a brighter and healthier future.
